Anti-Wrinkle Injections for Frown Lines, Forehead and Eyes: What to Expect

There's a reason anti-wrinkle injections are the most popular non-surgical treatment in the world. They're quick, they're precise, and when they're done well the result is quietly natural rather than obvious. Most people who come to see us aren't looking to freeze their face. They want the deep frown between the brows to soften, the horizontal lines across the forehead to relax, and the fan of creases at the corners of the eyes to fade a little. All of that is exactly what this treatment does best.

The appointment itself is short. You're usually in and out in around five to ten minutes, and back to your day straight afterwards. That brevity can make the treatment sound almost trivial, but the skill sits in the assessment beforehand: reading how your muscles move, deciding where to place each tiny amount of product, and dosing it so your expressions still look like yours. That's why we treat it as a medical procedure carried out by doctors, not a quick beauty appointment.

This guide walks through how anti-wrinkle injections work, the three areas the treatment is best known for, what happens on the day, and why the results often start lasting longer the more regularly you have them. If you've been curious but hesitant, it should give you a clear, honest picture of what to expect.

How anti-wrinkle injections actually work

The lines that appear when you frown, raise your brows or smile are called dynamic wrinkles. They form because the small muscles of the face contract thousands of times a day, and over the years that repeated folding of the skin leaves a crease behind. Think of the way a piece of paper stays lined after you've folded it back and forth.

Anti-wrinkle injections use a purified form of botulinum toxin, a protein that temporarily interrupts the signal between the nerve and the muscle. When that signal is softened, the treated muscle relaxes and stops pulling the skin into those familiar folds. The overlying skin gets a chance to settle and smooth out. Crucially, the effect is targeted. A well-placed treatment relaxes the muscles responsible for the lines you dislike while leaving the surrounding muscles free to move, so your face still expresses warmth, surprise and everything in between.

It's worth being clear about what the treatment can and can't do. It's excellent for lines caused by movement. Static lines, the ones etched in so deeply that they're visible even when your face is completely relaxed, respond less dramatically, though they often soften over time once the muscle underneath is calmer. For those deeper, at-rest lines, we sometimes suggest combining injections with treatments that work on the skin's surface and structure, such as skin boosters, Profhilo or resurfacing. The three classic treatment areas

Most first-time patients come in wanting one or more of the upper-face areas addressed. These are the regions where dynamic lines show earliest and where the treatment has the longest track record.

Frown lines between the brows

The vertical lines that appear between your eyebrows when you concentrate or frown are known as glabellar lines, or the "elevens". They're one of the most common reasons people book in, partly because a permanent frown can make you look cross, tired or worried even when you feel perfectly fine. Relaxing the muscles that draw the brows together softens those lines and tends to open up the whole expression. Many patients tell us people start asking whether they've been on holiday or had a good night's sleep, without ever guessing why.

Forehead lines

The horizontal lines that run across the forehead form when you lift your brows. Treating them well takes a careful hand, because the same muscle that creates those lines also holds the brows up. Over-treating it can leave the brows feeling heavy, so the aim is balance: softening the lines while keeping enough movement to hold everything comfortably in place. This is exactly why a conservative, well-judged dose matters more here than almost anywhere else on the face.

Lines around the eyes

Crow's feet are the little creases that fan out from the outer corners of the eyes when you smile or squint. Because the skin here is thin and delicate, it tends to show fine lines early. A few precisely placed injections relax the muscle that causes the creasing, softening the lines while leaving your smile intact. Treating this area often has the added benefit of making the eyes look a touch brighter and more rested.

These areas are frequently treated together, since balancing the upper face as a whole usually gives a more harmonious result than addressing one region in isolation. Beyond these three, anti-wrinkle injections can also be used for other concerns such as a gummy smile, downturned mouth corners, jaw slimming and excessive sweating, but the frown, forehead and eyes remain the classic starting point.

What happens during your appointment

The treatment itself is genuinely quick, but the part that matters most happens before any needle comes out. Your doctor will ask what's been bothering you and what you'd like to change, then watch your face at rest and in motion. Frown, raise your brows, squeeze your eyes shut, smile. This tells us how strong each muscle is and how it pulls, which in turn decides where the product goes and how much of it to use. No two faces are dosed identically, and a template approach is how you end up with that frozen look nobody wants.

Once the plan is agreed, the injections take only a few minutes. A very fine needle is used, and most people describe the sensation as a quick, small scratch or a tiny pinch rather than anything painful. No anaesthetic is usually needed. Including the consultation, you can realistically expect to be with us for around five to ten minutes of actual treatment time.

Afterwards you can head straight back to work or carry on with your day. There are a few simple things we ask you to do in the first 24 hours, and they're easy to follow.

  • Stay upright and avoid lying flat or bending over for around four hours.
  • Skip strenuous exercise, saunas, steam rooms and hot yoga for the rest of the day.
  • Leave the treated areas alone, so no rubbing or firm facial massage, and avoid facials for a couple of days.
  • Hold off on alcohol where you can, as it can add to any minor bruising.

Side effects tend to be minor and short-lived. You might notice small pinpoint marks, a little redness, or the occasional bruise, all of which settle quickly and are easily covered with makeup the following day. Serious problems are rare when the treatment is carried out by an experienced doctor who knows the anatomy well.

Results, the two-week review and how long it lasts

Anti-wrinkle injections don't work instantly, and that surprises some people. You'll typically start to notice the muscles relaxing from around three to five days, with the full effect settling in by about two weeks. That gradual onset is completely normal and part of why the change looks natural rather than sudden.

We ask every patient to come back for a review at around the two-week mark, and this appointment is an important part of the treatment rather than an optional extra. By then the product has fully taken effect, so we can see exactly how your muscles have responded and add a small top-up if any area needs a little more to sit perfectly. Everyone's muscles are slightly different in strength, and this fine-tuning is how we get the result just right for you.

Once settled, the effect generally lasts around three to four months. After that the muscle action gradually returns and the lines slowly begin to show again, which is the signal that it's time to think about your next treatment. There's an encouraging pattern worth knowing about here. Patients who keep up with their treatments regularly often find the results start to last longer over time. When a muscle is repeatedly kept relaxed, it tends to weaken slightly and the habit of frowning or over-lifting eases, so the lines take longer to reappear. It's not unusual for well-maintained, long-standing patients to stretch to every nine months, or even down to once a year, before they feel they need to come back. In other words, the treatment tends to reward consistency.

Is it right for you?

Anti-wrinkle injections suit a wide range of people, from those in their late twenties and thirties who want to slow the development of lines, to those who'd like to soften creases that have already set in. Some patients come purely for prevention, treating the muscles before deep lines have a chance to form. Others want to smooth what's already there. Both are entirely reasonable reasons to book.

The treatment isn't suitable for everyone. We don't treat during pregnancy or breastfeeding, and there are a small number of neuromuscular conditions and medications that mean it isn't appropriate. If you have an important event coming up, plan ahead: leave at least two to three weeks so the effect has settled and any minor bruising has faded. A good practitioner will also tell you when injections aren't the answer and something else would serve you better.

Common questions about anti-wrinkle injections

Will my face still move naturally?

Yes, when the treatment is dosed thoughtfully. The goal is to soften the lines you dislike while keeping natural, expressive movement. The frozen look tends to come from over-treatment, which is why careful assessment and a conservative approach matter so much. Does it hurt?

Very little. The needle is extremely fine and each injection takes a second or two. Most people describe a brief scratch or tiny pinch. There's no real recovery afterwards, and you can carry on with your day straight away.

How soon can I have it before an event?

Allow at least two to three weeks. The effect takes a couple of weeks to develop fully, and this window also gives any small bruise time to fade and lets us book your two-week review beforehand if needed. Leaving enough time means you'll look your best on the day rather than mid-transition.

What's the difference between anti-wrinkle injections and fillers?

They do different jobs. Anti-wrinkle injections relax muscles to soften lines caused by movement, such as frown and forehead lines. Dermal fillers add volume and structure, restoring plumpness or reshaping features. Many patients benefit from a combination, and we'll advise on the right balance for your face during your consultation.

What if I don't like the result?

Because the effect is temporary, nothing is permanent. If a result isn't quite what you hoped, your two-week review is the moment to adjust it, and any residual concern will naturally resolve as the product wears off over the following months. This temporary nature is one of the reasons the treatment feels so approachable.
