My patients ask me the same question all the time. Which treatment gives the biggest impact for the smallest change? It is a fair thing to want to know. Most people who walk into Wellface are not looking to be transformed. They want to look like themselves on a good day, rested and refreshed, without anyone being able to point at what they have had done.
For me, two treatments consistently come out on top. The first is the brow lift, done as part of an anti-wrinkle treatment, which I describe as opening up the eye and lifting the brow so you look less tired. The second, a very close runner-up, is a little lip filler. Not big lips. A small, subtle change to the mouth that makes the whole face look more rejuvenated.
Both are quick. Both are non-surgical. And both rely far more on judgement and placement than on the amount of product used. Here is a proper look at each one, how it works and what to expect if you decide to try it.
Why a subtle change reads as a rested face
There is a reason small adjustments around the eyes and mouth make such a difference. These are the two areas people look at when they talk to you. We read expression and age from the eyes first, and from the shape and fullness of the mouth second. When the outer brow drops and the eyes start to look heavy, the message the face sends is tiredness, even when you feel perfectly well. When the lips lose a little of their border and volume, the mouth can look flatter and the smile less lifted.
My approach is to treat the cause of that tired or flat appearance rather than to add something obvious. A brow that sits a millimetre or two higher at the tail changes how open and awake the eye looks. A lip that has its natural shape restored looks perkier without looking bigger. In both cases the aim is for people to notice you, not the treatment.
This is also why I am careful to frame these as my personal preferences. Every experienced injector has their own style and their own favourites, and the right plan always depends on the individual face in front of them. What follows is how I think about these two treatments, not a fixed recipe.
The Botox brow lift
A brow lift with anti-wrinkle injections is one of the most satisfying small treatments in aesthetics, precisely because it does so much with so little. There is no surgery, no cutting and no lengthy recovery. It comes down to a couple of tiny injections placed with real precision.
How it actually works
The position of your brow is decided by a tug of war between muscles. Some muscles pull the brow up, and others pull it down. The circular muscle around the eye, the orbicularis oculi, has an outer portion that pulls the tail of the brow downward. Over time that downward pull, combined with the frown muscles between the brows, can make the brow sit lower and the upper eyelid look heavier.
A brow lift uses a very small dose of botulinum toxin to relax the muscles that pull the brow down. Once that downward pull is softened, the muscles that lift the brow win the tug of war, and the tail of the brow floats up a touch. As I put it, it opens up the side of the eye and the brow, so you look and feel less tired. The lift is usually only a millimetre or two, but around the eyes a small movement reads as a big improvement.
Done well, it is often combined with treatment of the frown lines and forehead so the whole upper face is balanced. Skilled injectors also adjust the dose across the forehead carefully, because treating the wrong spot too heavily can drop the brow rather than lift it. This is exactly why placement matters more than volume, and why it should be done by an experienced medical injector.
What the session involves
The treatment itself is genuinely quick. I describe it as one or two teeny tiny injections at the tail of the brow, often as part of a wider anti-wrinkle plan. The needles used are very fine. Most people describe the sensation as a brief scratch or pinch rather than anything painful, and no numbing is usually needed.
Start to finish, an anti-wrinkle appointment with a brow lift element takes around fifteen to twenty minutes, including the consultation and assessment beforehand. You can drive yourself home and return to work the same day. There may be tiny raised bumps at the injection points for twenty minutes or so, and occasionally a small bruise, but there is no meaningful downtime.
When you will see results and how long they last
Anti-wrinkle injections do not work instantly. You will usually start to notice the brow settling into its new position from around day three to five, with the full effect visible at about two weeks. That two-week mark is when I and the team like to review, so any fine adjustment can be made. The lift typically holds for three to four months before the muscle activity gradually returns and you decide whether to repeat it. Many patients keep it up two or three times a year.
Subtle lip filler
Lip filler has a reputation problem, and most of that reputation is built on the small number of overdone results people see online. The reality in a doctor-led clinic is quite different. The majority of lip filler is subtle enough that nobody can tell it has been done. That is exactly the kind I favour, a small, subtle change to the mouth rather than big lips.
What it is and what it does
Lip fillers at Wellface are made from hyaluronic acid, a sugar molecule that occurs naturally in the body and is very good at holding on to water. When placed carefully in the lips, it does three things at once. It can enhance the shape and definition of the lip border, it restores volume that has been lost with age or that was never there to begin with, and it hydrates the tissue so the lips look smoother and healthier.
The key word is definition, not size. A well-placed small amount can lift the corners of the mouth, tidy a border that has softened, and smooth the fine vertical lines that appear on the upper lip over time. The result I am aiming for is a mouth that looks perky and refreshed, in proportion with the rest of your face.
How much is used and why less is more
Most people need far less filler than they imagine. A first treatment often uses a single syringe or less, with the product distributed in small amounts across the lip rather than concentrated in one spot. In smaller or thinner lips especially, building up gradually over more than one session tends to give a more natural, structured result and reduces the risk of filler spreading beyond where it should sit.
One of the reassuring things about hyaluronic acid filler is that it is reversible. If you were ever unhappy with a result, or in the rare event of a complication, the filler can be dissolved with an enzyme called hyaluronidase. That safety net is one reason these fillers are so widely used and trusted.
The appointment and aftercare
Before any product goes in, there is a proper consultation to talk through what you want and to assess your lips and the balance of your face. A numbing cream is usually applied first, and many lip fillers also contain a local anaesthetic, so the treatment is comfortable. The injections themselves take around fifteen to thirty minutes.
Swelling is normal and expected. Your lips will look their most swollen in the first two to three days, and this is the stage where people sometimes worry they have overdone it. Be patient. The swelling settles over roughly a week to ten days, and the true, softer result appears once it has fully gone down. A little bruising is possible too. For the first day or two it helps to avoid strenuous exercise, excessive heat such as saunas, and drinking through a straw. Staying hydrated and using an SPF lip balm helps your result look its best.
Depending on the product and your own metabolism, lip filler generally lasts somewhere between six and twelve months. Faster metabolisms and very active lifestyles tend to break it down a little sooner. Most patients come back for a top-up once or twice a year to maintain the look.
Who these treatments suit
The brow lift tends to appeal to people who feel they look tired or a bit heavy around the eyes, or who have noticed the outer brow starting to sit lower with age. It works beautifully alongside frown line and forehead treatment for an overall fresher upper face. It is a non-surgical option, so if you have significant hooding of the eyelids you may need a proper assessment to check whether it will give you enough, or whether something else is more appropriate.
Subtle lip filler suits people who want more shape, definition or hydration rather than dramatic volume. It is a good choice if your lips have thinned over the years, if your upper lip has developed fine lines, or if you simply want your smile to look a little more lifted. As with any treatment, filler is not suitable during pregnancy or breastfeeding, and a consultation is the place to talk through your medical history and expectations.
Both treatments share the same guiding principle. The best results come from understanding your individual features and tailoring the treatment around them, rather than applying a template. That is the whole point of seeing an experienced medical injector.
Common questions about brow lifts and lip filler
Will a Botox brow lift make me look surprised or frozen?
No, not when it is done properly. The lift I aim for is a millimetre or two at the tail of the brow, which reads as awake rather than startled. A permanently surprised or arched look, sometimes called a Spock brow, comes from incorrect placement or dosing, which is why the skill of the injector matters so much. This is a subtle refinement, not a dramatic change.
Can I have the brow lift and lip filler at the same appointment?
Often, yes, though it depends on your plan and preferences. Anti-wrinkle treatments and filler are different products doing different jobs, and many patients like to address the upper face and the lips together. I will talk you through whether to combine them or space them out, and how to sequence any review appointments.
Does lip filler hurt?
Most people find it very manageable. A numbing cream is applied first, and many lip fillers contain a local anaesthetic that takes effect as the treatment progresses. You will feel some pressure and the occasional sharp sensation, but it is generally described as uncomfortable rather than painful, and it is over quickly.
How soon will my lips look normal after filler?
Expect swelling for the first few days, at its peak around day two or three. Most of it settles within a week, with the final, natural result visible at roughly two weeks. It is worth booking treatment with this timeline in mind if you have an event coming up, so your lips have time to settle beforehand.
How do I know how much filler I actually need?
That is decided together at your consultation, based on your existing lip shape, the surrounding features and what you want to achieve. My preference is to start conservatively, often with a single syringe or less, and build gradually if needed. It is far easier to add a little more than to correct too much, and a measured approach gives the most natural result.
