Everything Lips: Filler, Lip Flips and Hydration Explained by Me
Lips get a lot of attention, and not always the good kind. For years the conversation has been dominated by images of overfilled, cartoonish results that put a lot of sensible people off the idea entirely. So it is worth saying clearly, right at the start, that this is not what modern lip treatments are about, and it is certainly not what we do at Wellface.
In a recent reel filmed at the clinic, I talked through the three treatments I reach for most often when someone wants to improve their lips. One adds volume and definition. One relaxes the muscle to lift the upper lip a little. And one does something many people do not realise is even possible, which is to hydrate and smooth the lips without changing their size at all. Each solves a different problem, and the trick is matching the treatment to what is actually bothering you.
Below, I have expanded on everything I covered, so you can walk into a consultation knowing roughly what you are looking for and, just as usefully, what you are not.
Lip filler, done with a light hand
Lip filler is a dermal filler made from hyaluronic acid, a sugar molecule that occurs naturally in the skin and holds on to water. When it is placed carefully into the lip, it restores structure and volume in the areas that need it, rather than simply making everything bigger.
That distinction matters. As I always say, the days of big duck lips are long gone, and they are not something I create. The goal is a subtle tweak that gives you back the definition and fullness you may have had in your twenties and lost over time. Lips thin gradually as we age. The border softens, the corners can turn down, and the little peaks of the Cupid's bow flatten out. Good filler addresses those specific changes so the result reads as a well-rested version of you, not as work.
What we can shape and improve
Volume is only part of the picture. A skilled injector is thinking about proportion, symmetry and the way the lip sits within the rest of your face. Common goals include defining the vermilion border so lipstick stops bleeding into fine lines, gently lifting the corners of the mouth, balancing an upper and lower lip that are slightly out of proportion, and enhancing the Cupid's bow. I single out the Cupid's bow as a particularly lovely area to refine, because a small amount of definition there can make the whole mouth look more sculpted without adding obvious bulk.
Most people are surprised by how little product it takes to make a difference. A conservative approach also means you can build gradually over time if you want to, rather than committing to a dramatic change in one appointment.
What a lip filler appointment involves
The treatment itself is quick, usually around twenty to thirty minutes once you have talked through the plan. Numbing cream is applied first, and most modern lip fillers also contain a local anaesthetic, so the discomfort is generally mild. Afterwards you can expect some swelling, and possibly a little bruising, for the first few days. Lips are vascular and reactive, so they can look fuller and more uneven immediately after treatment than they will once everything settles. Give it about two weeks before you judge the final result. Hyaluronic acid lip filler typically lasts somewhere between six and twelve months, depending on the product used, the area treated and how quickly your body metabolises it.
One of the reassuring things about hyaluronic acid filler is that it can be dissolved with an enzyme called hyaluronidase if you are ever unhappy or if there is a complication. It is not permanent, which gives both patient and doctor a useful safety net.
The lip flip: a little lift, no volume added
Sometimes the issue is not that the lips are too small, but that the upper lip disappears when you smile or that it sits flat against the teeth. This is where the anti-wrinkle lip flip comes in, and it is one of my favourite quiet tricks.
A lip flip uses a tiny dose of anti-wrinkle injection (the same botulinum toxin used for expression lines) placed into the muscle that circles the mouth, the orbicularis oris. By relaxing the very edge of that muscle along the upper lip, the lip rolls, or flips, gently outwards. More of the pink of the lip shows, and the result is a slightly fuller, perkier appearance. Crucially, no filler is involved and no volume is added. You are simply revealing a little more of the lip you already have.
Who the lip flip suits
I describe it as ideal for two groups. First, people who are not sure filler is for them, or who want the most understated result possible. Second, people who have already had a small amount of filler and want a second little pick me up on top, that final flourish that makes the upper lip look lifted. It can also help if you show a lot of gum when you smile, because relaxing the muscle reduces how far the upper lip lifts.
Because the doses are so small, this is a very subtle treatment. It will not transform a thin lip into a full one, and it is not a substitute for filler if volume is what you genuinely need. Think of it as fine-tuning rather than filling.
How long a lip flip lasts
The effect tends to appear within a few days to a week and generally lasts around two to three months, which is shorter than anti-wrinkle treatment elsewhere on the face because the muscle around the mouth is used constantly for talking, eating and drinking. Many people top it up a few times a year, often alongside their other treatments. For the first day or so it is sensible to avoid anything that involves strong pursing or straws while things settle, and I will talk you through the specifics.
Anti-wrinkle injections for lines around the mouth
Hydration and volume are only two of the three concerns people bring to the clinic. The third is fine lines, and the mouth is one of the earliest places they show. Those vertical lines above the top lip, sometimes called smoker's lines even in people who have never smoked, come from a combination of repeated muscle movement, sun exposure and the natural loss of collagen and hydration over time.
Carefully placed anti-wrinkle injections can soften these lines by easing the pull of the muscle that creates them. The approach here is gentle and conservative, because the muscles around the mouth do important work and you never want to affect speech or the way you eat. Often the best results come from combining a small amount of anti-wrinkle treatment with hydration, so you are relaxing the lines and improving the quality of the skin at the same time rather than relying on one alone.
Lip hydration with Volite
The treatment I would say has become genuinely popular of late is lip hydration, and it is the one people tend to know least about. So many patients arrive with lips that feel depleted, dry and criss-crossed with fine lines. They reach for lip balm constantly and it never quite fixes the problem. The instinct is to assume they need filler, but often they do not. What they need is moisture and better skin quality.
Volite is a hyaluronic acid injectable designed for exactly this. Rather than being placed as a bolus to add structure, it is delivered as a series of tiny microinjections that spread hydration throughout the tissue. The hyaluronic acid draws in and holds water, which plumps the lip from within, smooths the surface, softens fine lines and improves elasticity. What it does not do is add obvious volume or change your shape. Your lips look like your lips, just healthier, softer and more supple.
Why lip hydration has taken off
Part of the appeal is that it feels natural and low-key. I describe it as stunning for the lip balm lovers, the people who are forever moisturising and want a longer-lasting solution to dryness and texture. It is also a good entry point for anyone curious about lip treatments but nervous about filler, because there is no risk of looking overdone. And the results have real staying power. A hydration treatment like this can last around six to nine months, which is considerably longer than any cream or balm can offer.
What to expect from a Volite session
The appointment is straightforward. After numbing cream, the product is placed with a fine needle across the lip in a series of small injections. You may notice tiny bumps immediately afterwards, along with some redness or mild swelling, and these settle over the following day or two. There is very little downtime, and most people are back to normal quickly. Because the effect builds as the skin rehydrates, you will notice the lips looking and feeling smoother over the first couple of weeks.
How to choose the right lip treatment
With three good options on the table, the sensible question is which one is right for you, and the honest answer is that it depends entirely on what is bothering you. If your lips have lost volume and definition and you want to restore shape, filler is the natural choice. If your upper lip vanishes when you smile and you want a subtle lift without adding volume, a lip flip is worth a conversation. If your lips are dry, thinning in texture and lined but you are happy with their size, hydration with Volite is likely to be the answer.
These treatments are not mutually exclusive. Many patients combine them, for example a modest amount of filler for structure paired with a lip flip for a little extra lift, or hydration alongside anti-wrinkle treatment to tackle fine lines and dryness together. As I often say, whether it is filler and a little lip flip, just hydration, or all of them, the plan is built around your lips and your goals. That is what a consultation is for.
Common questions about lip treatments
Will lip filler make me look overdone?
Not when it is placed conservatively. At Wellface my aim is definition and balance, not size for its own sake. The results should look like your own lips on a good day. If you ever want to reverse hyaluronic acid filler, it can be dissolved, which is a genuine reassurance for anyone nervous about the idea.
Does a lip flip actually add volume?
No. A lip flip uses a small dose of anti-wrinkle injection to relax the muscle so the upper lip rolls outward slightly, revealing a little more of the lip you already have. It is a subtle lift rather than a fill, which is exactly why it suits people who want the most understated result or a finishing touch on top of filler.
How is Volite different from lip filler?
Both use hyaluronic acid, but they work in different ways. Filler is placed to add volume and structure. Volite is delivered as tiny microinjections that spread through the tissue to hydrate the lip, smooth texture and improve elasticity without changing the size or shape. If your main complaints are dryness and fine lines rather than lost volume, Volite is usually the better fit.
Do these treatments hurt?
Most people find them very manageable. Numbing cream is applied first, and lip fillers generally contain a local anaesthetic as well. You may feel small pinches and some pressure, and afterwards a degree of swelling or occasional bruising is normal, particularly with filler. Any discomfort tends to be short-lived.
How long before I see the final result?
With filler, allow around two weeks for swelling to settle before judging the shape. A lip flip develops over several days to a week. Hydration builds gradually as the skin rehydrates over the first couple of weeks. In each case the immediate appearance is not the finished result, so a little patience pays off.
Come and talk lips with me
The best way to work out which of these treatments suits you is to sit down with a doctor who can look at your lips, listen to what you would like to change and be honest about what will and will not help. I offer that guidance without any pressure, and often my recommendation is more modest than people expect.
If you would like fuller, more defined, better hydrated lips that still look completely like you, I would love to see you. Wellface has clinics in Cobham, Esher and Wimbledon, and you can book a consultation with me online or by phone. Pop in, let's talk lips, and I will help you decide on the right next step.
