Exosomes Explained: The Next Step in Regenerative Skin and Hair Treatments

I get asked about exosomes almost every week now, and I am more than happy to talk about them. If you have followed Wellface for a while, you will have heard me mention this treatment as the newest generation of regenerative medicine. It sounds like something out of a science lab, and in a sense it is, but the idea behind it is surprisingly simple.

Exosomes are messengers. They carry the instructions your skin cells and hair follicles need to repair, rebuild and behave more like they did when you were younger. For anyone who already loves PRP, the platelet-rich plasma treatment that uses your own blood to stimulate renewal, exosomes are the natural next thing to try. They work on the same principle of switching on your body's own repair mechanisms, but they deliver a known, consistent dose of the growth factors and proteins that do the heavy lifting.

This article walks through what exosomes actually are, how they help with hair thinning, dull skin and the early signs of ageing, what a session at the clinic involves, and how they compare with the treatments you may already know. I have a firm rule at Wellface: nothing goes into a patient's skin unless my team and I have tried and tested it first. Exosomes passed that test.

What exosomes actually are

Exosomes are tiny vesicles, nano-sized packages, that cells release to communicate with one another. Think of them as text messages sent between cells. They are much smaller than a single cell, roughly 30 to 150 nanometres across, and they carry a cargo of growth factors, amino acids, peptides, proteins and genetic signalling material.

When I describe them as loaded with billions of growth factors, I mean that each dose delivers a huge concentration of the exact signals your tissue uses to repair itself. In aesthetic medicine, the exosomes used are typically derived from cultured stem cells in a laboratory, then purified so that what remains is the beneficial messaging without any living cells. That distinction matters. You get the regenerative instructions that stem cells produce without introducing the cells themselves, which keeps the treatment more stable, more consistent and easier to standardise.

Once they reach your skin or scalp, exosomes bind to your own cells and release their contents. This nudges the cells into action: making more collagen, forming new small blood vessels, calming inflammation and generally repairing tissue. The effect is not a quick surface fix like a moisturiser sitting on top of the skin. It happens deeper, at the level where cells decide how to behave.

How exosomes differ from PRP

PRP has been a mainstay of regenerative aesthetics for years, and I am a genuine fan of it. The process involves taking a small sample of your blood, spinning it in a centrifuge to concentrate the platelets and plasma, then re-injecting that concentrate into the skin or scalp. Those platelets are packed with your own growth factors, and they set off a healing response wherever they are placed.

The catch with PRP is that the result depends on you. If you are healthy and well, your plasma is likely to be rich and effective. If you are run down, unwell, older, or your blood quality is not at its best on the day, the concentration of useful growth factors can vary. You are, in effect, working with whatever your body hands over that morning.

Exosomes remove that variability. Because they are prepared to a known specification, every dose contains a measured, reliable quantity of the growth factors, amino acids and proteins being targeted. I often frame it this way: with PRP I am estimating the quality of what I am injecting, whereas with exosomes I know precisely what is going in. That consistency is a big part of why the results have been so impressive.

It is worth saying that this is not a case of one replacing the other. Many practitioners see PRP and exosomes as complementary tools, and both have their place. Exosomes simply give us a more predictable, high-concentration option, particularly useful when someone's own plasma might not be at its strongest.

Exosomes for hair loss and thinning

Hair thinning is one of the most common concerns I hear about, from both women and men, and it is one of the areas where exosomes are proving especially useful. Pattern hair loss and general thinning tend to involve follicles that gradually shrink, spend less time in the active growth phase and produce finer, weaker strands over time.

Exosomes work on the scalp by delivering signals directly to the cells that govern the hair cycle. They help wake up dormant follicles and encourage them to shift from the resting phase back into the growth phase. They also improve the local environment around the follicle by calming inflammation and supporting better blood supply, which gives each hair a healthier base from which to grow. The aim is thicker-looking, stronger hair and a fuller appearance overall.

For hair, exosomes are usually applied to the scalp in combination with microneedling, which creates tiny channels that let the exosomes reach the follicles, or delivered by fine injections. Because the concern builds up over years, results are gradual rather than instant. Most people are looking at a course of sessions and several months before the picture really improves, which is entirely normal for any regenerative hair treatment.

Exosomes for dull skin and glow

If your skin has lost its brightness and looks tired no matter how much you sleep, exosomes can help restore that lit-from-within quality. Dullness is often a sign that cell turnover has slowed, hydration has dropped and collagen production is not what it once was. Exosomes address all three by prompting the skin's cells to renew, repair the barrier and hold on to moisture more effectively.

The most popular way to deliver them for skin is straight after microneedling or a laser treatment. This is where timing genuinely matters. Once micro-channels are created in the skin, they only stay open for a short window, roughly ten to fifteen minutes, before they begin to close. Applying the exosomes within that window lets them penetrate deeply rather than sitting on the surface. This is exactly why exosome application happens in the treatment room and not as something you take home in a bottle.

Layering exosomes onto microneedling, or onto devices such as the Secret Pro or CO2 laser, does two things at once. It speeds up recovery, so redness and swelling settle faster, and it enhances the end result by pouring regenerative signals into skin that is already primed to respond. Patients tend to notice smoother texture, more even tone, better hydration and that fresh, healthy glow in the weeks that follow.

Exosomes for ageing prevention

Prevention is where a lot of the smartest aesthetic decisions are made, and exosomes fit neatly into that thinking. Rather than waiting for deep lines and slackness to appear and then trying to correct them, you can support your skin's own collagen and elastin production while things are still in good shape.

By keeping fibroblasts, the cells that manufacture collagen, active and working, exosomes help maintain firmness and resilience over time. The improvements build gradually and continue as collagen renews, which suits people who want steady, natural-looking maintenance rather than a sudden change. This makes exosomes a sensible option for anyone in their thirties and forties who wants to look after their skin's structure now, as well as for those who already have early signs of ageing and want to slow the pace.

Because exosomes work on skin quality and health rather than adding volume or freezing movement, they sit comfortably alongside other treatments. Many of my patients combine them with anti-wrinkle injections, fillers or Profhilo as part of a broader plan, using each treatment for what it does best.

What a session involves and what to expect afterwards

A typical exosome treatment starts with a consultation so I can understand your concern, whether that is your scalp, your skin or both, and confirm the treatment is right for you. On the day, the area is cleansed and, for skin, a numbing cream is usually applied to keep things comfortable.

The exosomes are then delivered using microneedling, a laser, or fine injections, depending on your goals. The whole appointment is generally quick, often under an hour including preparation. Downtime is modest. You can expect some redness and mild sensitivity for a day or two, similar to how skin looks after microneedling, and this settles on its own.

Aftercare is straightforward. Keep the area clean, avoid heavy make-up, strenuous exercise and heat, such as saunas and hot showers, for the first day or so, and be diligent with sun protection while your skin recovers. I will give you specific guidance based on the treatment you had. Because this is a regenerative treatment that works with your body over time, a short course spaced a few weeks apart usually gives the best results, followed by occasional maintenance.

Common questions about exosomes

Are exosomes safe?

Exosome treatments used in aesthetics are prepared in laboratories to strict standards and contain no living cells, which makes them stable and well tolerated. As with any injectable or microneedling-based treatment, the most common effects are temporary redness and mild swelling. How soon will I see results?

Regenerative treatments reward patience. Some people notice their skin looking brighter and feeling smoother within a couple of weeks, but the deeper benefits, such as improved collagen, firmer skin or fuller-looking hair, develop gradually over the following months. A course of sessions generally produces a more noticeable and lasting outcome than a single treatment.

Can exosomes be combined with other treatments?

Yes, and they often are. Exosomes pair particularly well with microneedling and with laser resurfacing, where they enhance the result and help the skin recover faster. They also complement injectables like anti-wrinkle treatment, fillers and Profhilo, because they improve the quality and health of the skin rather than competing with those treatments.

Are exosomes better than PRP?

I will advise which suits you, and in some cases a combination makes sense.
