Lip Fillers and Profhilo: The Pairing That Freshens the Whole Face
Some of the most rewarding appointments start with a patient saying something like "I trust you, do what you think." It sounds relaxed, but it carries a lot of weight. When someone hands over that kind of trust, the job is to read the face honestly, suggest only what genuinely helps, and hold back on anything that would tip a natural result into an obvious one.
That was the brief behind a recent treatment at Wellface. The patient wanted two things. She wanted her lips "sorted," and she wanted to look a bit fresher across the whole face. Not dramatically different. Just rested, lifted and a little more even. After assessing her lips and her skin, the plan came together quickly: a modest amount of lip filler to add volume and balance, paired with a course of Profhilo to rehydrate and lift the skin. The reaction when she saw the result in the mirror said the rest.
This pairing comes up often in the clinic, and for good reason. Lips draw the eye, but they never sit in isolation. When the skin around them looks tired or lax, even beautifully treated lips can look slightly out of context. Treating both at once, in the right order, is what makes the finished result read as "you, on a good day" rather than "you, but done."
Why lip filler on its own is only half the picture
Lip filler is one of the most requested treatments in aesthetics, and it is easy to see why. Modern hyaluronic acid fillers can add volume, define the lip border, correct asymmetry and soften fine lines around the mouth, all in a short appointment. Hyaluronic acid is a sugar molecule your body already makes, which is part of why these fillers feel soft and integrate well with your own tissue. If you are ever unhappy with the result, the same family of products can be dissolved with an enzyme called hyaluronidase, which is a reassuring safety net that few other treatments offer.
The catch is that lips are a feature, not a standalone project. A face that has lost a little firmness, or looks slightly dull and dehydrated, will not be fixed by lips alone. In some cases, adding volume to the lips without addressing the surrounding skin can even make the rest of the face look more tired by comparison. That is why our doctors assess the whole face before they touch a syringe. The question is never simply "how much filler," it is "what does this face actually need to look fresher."
In this patient's case, the answer was less filler than many people expect, plus attention to the skin. Just over half a millilitre went into the lips. That is a genuinely conservative amount, chosen to add fullness and even out a small asymmetry without changing her character. The freshness came from Profhilo.
How the doctors decided what to treat, and in what order
One detail from this consultation is worth drawing out, because it shows how careful assessment prevents overtreatment. The patient had a slight unevenness in her lips when she smiled, and a small amount of gum showing on one side. There is a treatment for a prominent "gummy" smile, using anti-wrinkle injections to relax the muscles that lift the upper lip too high. It is very effective in the right patient.
But she was not that patient. Her upper lip did not travel dramatically upwards when she smiled, and she did not have a lot of gum on display. Rather than reaching for the gummy smile technique straight away, the doctor recommended balancing the lips with filler first. Adding volume and evening out the shape can itself soften how much gum shows when someone smiles, because the upper lip has a little more substance to it. Sure enough, after the filler, there was visibly less gum on show when she smiled, without a single unit of anti-wrinkle injection near the area.
This is the kind of decision that separates a considered plan from a menu order. Treat the simplest, most reversible thing first. See how the face responds. Only add more if it is genuinely needed. It is also why the eye area was left for Profhilo rather than being tackled with anti-wrinkle injections at that first visit. When skin quality and hydration are the underlying issue, improving the skin often does more than freezing a muscle.
What Profhilo actually does
Profhilo is not a filler in the usual sense, even though it is made from hyaluronic acid. It contains one of the highest concentrations of hyaluronic acid on the market, delivered in a way that lets it spread smoothly through the skin rather than sitting in one spot to add volume. Once injected, it does two jobs. It draws water into the tissue for an immediate boost in hydration, and it stimulates the skin to produce more of its own collagen and elastin over the following weeks. The result is skin that looks plumper, feels firmer and has a subtle glow, without any change to your facial shape.
Because it works on skin quality rather than volume, Profhilo is often described as bio-remodelling rather than filling. It is a favourite for people in their thirties, forties and beyond who feel their skin looks a little flat, crepey or tired, but who do not want the fuller look that traditional filler gives. In this patient's case, it was chosen specifically to freshen the eye area and give the face an overall lift, which is exactly the kind of soft, all-over improvement it does best.
The five-point technique
Profhilo is delivered using a method known as the BAP technique, short for Bio Aesthetic Points. Instead of many small injections dotted across the face, the product is placed at just five carefully chosen points on each side. These points are selected because they sit a safe distance from major blood vessels and nerves, and because they let the product spread evenly across the cheeks, jawline and surrounding areas. Ten little injections in total, and the highly fluid gel does the rest as it disperses under the skin.
That is exactly what the patient in the reel experienced. Five points on one side, five on the other, and a treatment that is genuinely quick and comfortable. For most people the appointment is done well within half an hour.
A course, not a one-off
Profhilo is usually given as a short course. The standard protocol is two sessions spaced about four weeks apart, which gives the skin the ideal stimulus to start remodelling. After that, most people maintain their results with a top-up every six to nine months. You will often see a fresher, more hydrated look within a week or two, with the firming, collagen-building benefits continuing to develop over the following months.
What the appointment feels like, and the downtime
Neither of these treatments involves surgery or significant recovery, which is part of their appeal. Lip filler is usually done with a numbing cream and often a local anaesthetic within the product itself, so it is very tolerable. Expect some swelling straight afterwards. Lips can look larger and feel firm for the first day or two, because hyaluronic acid attracts water and the injections themselves cause a little inflammation. Most of that settles within three to seven days, and the true final result appears at around two weeks once everything has calmed down and the filler has integrated.
Profhilo tends to be even gentler on downtime. You may notice small raised bumps at the injection points, a bit like tiny insect bites, but these usually flatten out within a few hours to a day as the product disperses. A little redness or bruising is possible, as with any injection, but most people head straight back to their day.
The patient in the reel mentioned going red in the face, which is a completely normal, temporary flush after injections. It fades quickly and is nothing to worry about.
Simple aftercare that protects your result
Good aftercare is mostly about being sensible for a day or two. For lips, that means keeping the area clean, avoiding heavy exercise, saunas and hot yoga for around 48 hours, going easy on alcohol, and trying not to poke or massage the lips while they settle. Sleeping with your head slightly elevated on the first night helps limit swelling. A gentle, fragrance-free lip balm keeps everything comfortable, and once healed, a lip balm with SPF helps protect your result, because UV exposure can break hyaluronic acid down faster.
For Profhilo, the advice is similar. Avoid make-up over the injection points for the rest of the day, skip intense exercise and heat for a day or so, and keep skincare gentle. Both treatments reward patience. Judge the final look at two weeks, not two hours.
Who this pairing suits
The lip filler and Profhilo combination works particularly well for someone who wants a subtle refresh rather than an obvious transformation. If your lips have lost a little volume or definition, or have always been slightly uneven, a small amount of filler can restore balance. If your skin looks flat, dull or a touch lax, especially around the eyes and cheeks, Profhilo addresses the quality of the skin itself. Together they cover both volume and freshness, which is why the finished result tends to look so natural.
It is a popular choice across a wide age range, but it is especially suited to people who are starting to notice the first signs of tired-looking skin and want to stay ahead of it without committing to anything dramatic. As with everything at Wellface, suitability is decided at a face-to-face consultation. Not everyone needs both treatments, and part of a doctor's job is to say so when one alone would do.
Common questions about lip fillers and Profhilo
How long do lip fillers last?
Most hyaluronic acid lip fillers last somewhere between six and twelve months, depending on the product used, your metabolism and lifestyle factors like sun exposure and how active you are. Lips are a mobile, well-supplied area, so filler there can break down a little faster than in some other parts of the face. Many people choose a small top-up once or twice a year to maintain their look.
Can I have both treatments on the same day?
In many cases the two can be planned within the same overall course, and your doctor will advise on timing. Sometimes it makes sense to space them slightly so that swelling from one does not make it harder to judge the other, and so aftercare stays simple. Your practitioner will map out the sequence at your consultation based on your priorities, just as happened in this case where the lips were treated and Profhilo formed part of the wider freshening plan.
Will lip filler look obvious?
Not when the amount and placement are right. The patient here had just over half a millilitre, which adds a soft, believable fullness rather than a noticeable pout. Our doctors would always rather start conservative and add a little more later than overfill in a single sitting. Filler can also be dissolved if you are ever unhappy, which takes a lot of the anxiety out of the decision.
Does Profhilo add volume like filler?
No, and that is the point. Profhilo improves the quality of the skin, its hydration, firmness and glow, without changing your facial contours. It will not give you fuller cheeks or plumper lips. If you want structural volume, that is a job for filler. If you want your existing skin to look healthier and more lifted, Profhilo is the better tool. The two are complementary rather than interchangeable.
Is there much bruising?
Bruising is possible with any injectable but is often minor. You can reduce the risk by avoiding alcohol and, where medically appropriate, blood-thinning supplements for a day or two beforehand. Any bruising that does appear usually fades within a week and covers easily with make-up once the skin has settled.
Book a consultation at Wellface
The reason this treatment worked so well was not a single product. It was the assessment behind it: treating the lips conservatively, choosing Profhilo for freshness, and leaving the anti-wrinkle injections aside because the face did not need them. That is the kind of honest, doctor-led planning our patients come back for.
If you would like fuller, better-balanced lips, fresher-looking skin, or simply an expert opinion on what would actually make a difference for your face, our doctors would be glad to talk it through with you. You can book a consultation at Wellface in Cobham, Esher or Wimbledon, and we will build a plan around what suits you rather than a fixed menu. Come in, tell us what is bothering you, and let us show you what a considered, natural result looks like.
