The Lip Flip Explained: Fuller-Looking Lips Without Adding Volume
There's a particular kind of result that makes people do a double take. The lips look fuller, the smile shows more of that soft pink border, and yet nothing looks done. No obvious volume, no "trout pout", just a mouth that seems a little more open and expressive than before. That's the lip flip, and it's one of the quietest, most underrated treatments in the injectable toolkit.
We shared a recent lip flip transformation on Instagram because it captured exactly what the treatment does well. Our patient already had lovely full lips, so the goal was never to make them bigger. It was to reveal a little more of the upper lip when she smiled, keeping everything looking natural and balanced. A few weeks later, the difference was clear: more upper lip on show, a softer smile, and not a hint of anything artificial.

What a lip flip actually is
A lip flip is a small anti-wrinkle injection treatment for the upper lip. Rather than injecting filler into the body of the lip to add volume, a tiny amount of muscle relaxant is placed into the muscle that runs around the mouth, the orbicularis oris. This is the ring of muscle you use to purse your lips, whistle or drink through a straw.
In many people, that muscle pulls the upper lip inward and slightly downward, especially when smiling or talking. So the pink part of the lip, the vermilion, tucks under and hides. When we relax the very edge of that muscle with a few precise units, the upper lip gently rolls, or "flips", outward. More of the pink shows. The lip looks fuller and more relaxed, even though not a single drop of volume has been added.
That's the key thing to hold on to. A lip flip changes the position of the lip, not its size. It's an illusion of fullness created by muscle relaxation, and for the right person it's remarkably effective.
How it differs from lip filler
People often use "lip flip" and "lip filler" as if they were the same treatment, but they work on completely different principles. Lip filler is a hyaluronic acid gel injected directly into the lips to add genuine volume, shape and definition. It plumps the tissue, can smooth fine lip lines, and gives an immediate visible change that typically lasts somewhere between six and twelve months.
A lip flip uses botulinum toxin, the same product family used for forehead and frown lines, to relax a muscle. There's no added volume, the change is subtler, and it lasts a shorter time. Filler is the answer when someone wants more lip. A flip is the answer when someone has enough lip but wants more of it to show, or wants to soften a smile that pulls the top lip thin.
The two are not rivals. In fact they pair beautifully, and many patients eventually have both. But if you're new to lip treatments and want the gentlest possible starting point, the flip is often where we begin.
The treatment itself
The appointment is quick, which surprises a lot of first-timers. After a consultation to check that a flip is right for you and to agree on the look you're after, the injections take only a few minutes. In the reel you can hear the practitioner talking through it as she works, placing a small number of tiny injections, usually four points, along the upper lip border.
The amount of product used is very small. A lip flip generally uses only around four to six units of muscle relaxant, far less than a typical anti-wrinkle treatment for the forehead. That small dose is deliberate. The aim is to relax just enough of the muscle edge to let the lip roll outward, without weakening the mouth so much that speaking, eating or drinking feels odd.
Discomfort is minimal. Most people describe the injections as a few quick pinches rather than anything painful. There's no need for extensive numbing in most cases, and you can walk straight back out into your day afterwards. There's no meaningful downtime, no dressings and nothing to hide.
When you'll see the result
This is the part that catches people out, so it's worth being clear. A lip flip is not an instant result. Because it relies on muscle relaxant, it works on the same timeline as any anti-wrinkle injection. As we explain to every patient, muscle relaxant treatments need roughly ten days to two weeks to take full effect.
So don't judge your lips the day after treatment. In the first few days you may notice very little. By around a week the flip starts to become visible, and by the two-week mark you're seeing the finished effect. That's exactly why the transformation in our reel was filmed a few weeks on, once the treatment had settled and the true result had appeared.
Who it suits, and who it doesn't
The lip flip is a lovely option for people who feel their upper lip disappears when they smile, or who show a thin strip of top lip and would like a touch more to show. It's also popular with anyone who wants a very natural enhancement and feels nervous about committing to filler. Because it's temporary and subtle, it makes an ideal first step into lip treatments.
It also tends to appeal to people who already like the size of their lips. Our patient in the reel is a good example. She had naturally full lips, so filler would have risked tipping the balance. A flip simply let more of what she already had come forward when she smiled.
There are limits, though, and an honest consultation matters. If you have very thin lips and you're hoping for a real increase in volume, a flip alone probably won't get you there, and filler would be the better route. A flip works with the muscle, not the tissue, so it can only reveal what's already present. Some people also notice their lips feel a little different for the first week or so, perhaps slightly stiff, or a touch of awkwardness sipping through a straw or pronouncing certain sounds. This settles as the treatment beds in, and using a smaller, well-placed dose keeps it to a minimum.

How long it lasts and looking after your results
A lip flip is temporary. Because the lips are among the busiest muscles in the body, constantly moving as we talk, eat and express ourselves, the effect tends to wear off faster than muscle relaxant does elsewhere on the face. Most people find a flip lasts somewhere in the region of two to three months. Some see closer to six or eight weeks, others a little longer. To keep the look going, treatments are usually repeated every couple of months.
That relatively short lifespan is not really a drawback. It's part of the appeal. If you love the result, a quick top-up maintains it. If you decide the look isn't for you, it simply fades and your lips return to exactly how they were before. Few aesthetic treatments give you that kind of low-commitment trial run.
Aftercare is straightforward. As with any muscle relaxant treatment, it helps to stay upright for a few hours afterwards and to avoid rubbing or massaging the area so the product stays exactly where it was placed. Give strenuous exercise, saunas and hot yoga a miss for the rest of the day. Beyond that, there's very little to think about. Keeping your lips hydrated and using a balm with SPF is good general practice, and it helps your lips look their best whatever treatment you've had.
Common questions about the lip flip
Does a lip flip hurt?
Not much. The injections are small and shallow, and most people describe them as a quick pinch or two rather than genuine pain. The whole thing is over in a few minutes. If you're particularly sensitive, we can discuss numbing options at your consultation, but many patients find they simply don't need it.
Will it make me look overdone?
No, and that's rather the point. A lip flip adds no volume, so it can't create the overfilled look people worry about. It reveals a little more of your natural lip when you smile and softens the way your top lip sits. Done with a small, careful dose, the result is subtle enough that people tend to notice you look well rather than notice you've had anything done.
Can I have a lip flip and filler together?
Yes, and it's a popular combination. Filler builds volume and shape, while the flip lets more of the upper lip show and can soften a gummy smile. Used together, and in the right proportions, they can give a fuller yet still natural result. Why didn't I see a result straight away?
Because a lip flip uses muscle relaxant, it takes time to work.
