Men's Aesthetics at Wellface: A Doctor-Led Approach to Looking Less Tired
A patient came in a few months ago and said something we hear from men more often than you might think. He looked tired. He didn't quite recognise himself in the mirror. He felt he'd aged over the last couple of years but couldn't put his finger on exactly what had changed. There was no single feature he wanted to fix, just a general sense that the reflection didn't match how he felt inside.
That kind of consultation is one of the most rewarding parts of what we do. There's rarely one magic answer. What works is a careful look at the whole face, an honest conversation about what's realistic, and a plan that treats a few things gently rather than one thing dramatically. For this gentleman that meant a small rejuvenation package: anti-wrinkle injections to soften the frown and forehead, and dermal filler to rebuild structure through the cheeks, chin and jaw. The result was a stronger, more contoured, better-rested version of himself, without ever tipping into looking "done".
Male aesthetics has quietly become one of the busiest areas of our clinics in Cobham, Esher and Wimbledon. Below is how we think about treating men, what a package like this actually involves, and the wider menu of options, from jawline sculpting to scalp health, that helps men look and feel more like themselves.
Why treating men is different
The goals men bring to a consultation tend to be practical. Look less tired. Look less angry or stern. Look like you did a few years ago, not like someone else entirely. Almost nobody wants an obvious change that colleagues will notice and comment on. They want to look well-rested and to keep the features that read as masculine.
That has real consequences for technique. Male faces generally have heavier brows, flatter and more angular cheeks, a wider and squarer jaw, and a stronger, more forward chin than female faces. Skin is thicker and carries more blood supply, which is why men can bruise a little more and often need slightly more product to see the same effect. When we treat, we're working with all of that in mind.
Take the forehead. With anti-wrinkle injections we're aiming to soften the deep frown and the horizontal lines, not to freeze the whole upper face into a glassy, expressionless surface. On a man that flat, over-treated look reads as unnatural very quickly. The skill is in dosing and placement that relaxes the muscles pulling the brow into a scowl while leaving enough movement to keep a normal, engaged expression. That's exactly what we did for the gentleman above. His forehead is smoother and softer, but he still looks like himself and still looks masculine.
Building the structure back: cheeks, chin and jaw
The bigger shift for this patient came from dermal filler, used not to plump but to rebuild structure. As we age we lose bone density and fat in predictable places, and the face starts to look flatter and to sag slightly. Restoring support in the right spots lifts and defines the whole face.
Cheeks that support rather than round
On men we place cheek filler to create a flatter, more angular contour and to give the midface a bit of lift and support. It's a very different aesthetic from the soft, rounded "apple" cheek many women ask for. A touch of definition high on the cheekbone reads as strength, and it also helps hold up the tissue lower down, which is part of why the overall face looked more refreshed.
A stronger chin makes the biggest difference
From the side, this gentleman had what we'd describe as a weak, slightly set-back chin. In dental terms that's a class 2 skeletal pattern, where the lower jaw and chin sit further back than the upper. It's extremely common and it has an outsized effect on how the profile reads, often making the nose look larger and the neck look less defined than they really are.
By building the chin forwards and downwards with filler, we gave him a stronger, more sculpted profile almost immediately. Chin work is often the single most transformative thing we do for men, precisely because a projecting chin is such a strong marker of a masculine face. It balances the nose, tidies up the transition into the neck and instantly makes the jaw look more defined.
Jawline definition to finish the frame
We then added a little filler along the jawline itself and at the angle of the jaw, near the back where the jaw meets the ear. Sharpening that angle and reinforcing the line of the jaw creates the "frame" that defines the lower face. For men we're usually going for a clean, square, slightly wider angle rather than the tapered look. Combined with the chin work, this gave him the chiselled definition he was after and pulled the whole result together.
The nice thing about this staged approach is that each element is subtle on its own. Nobody can point to the change. They just think he looks well.
Tear troughs and the tired-eye complaint
The single most common phrase men use in consultation is "I look tired." Very often the eyes are a big part of that. Hollowing under the eye, sometimes with a bit of shadow or a groove between the lid and cheek, can make even a well-rested person look exhausted.
Tear trough filler treats this by placing a small amount of a soft, hydrating filler deep under the eye to fill the hollow and smooth the transition into the cheek. Done well it's remarkably natural, and for the right candidate it can take years off the appearance in minutes. It is a genuinely technical area, close to delicate structures, which is why it should only be done by an experienced medical injector after proper assessment. Not everyone is suitable. If there's significant puffiness or loose skin rather than hollowing, filler can make things worse, and we'll say so. When it's the right treatment, though, it's one of the most satisfying for that "why do I always look shattered" concern.
Beyond the face: hair, fat and pelvic health
Rejuvenation for men doesn't stop at the face, and a proper package often looks at a few things at once.
Keravive for the scalp
Hydrafacial Keravive is a treatment for the scalp rather than the skin. It uses the same technology as the facial to deeply cleanse and exfoliate the scalp, clearing away build-up and dead skin around the follicles, then delivers a serum of growth factors and skin proteins designed to nourish the scalp and support healthier, fuller-looking hair. You go home with a take-home spray to continue the effect between sessions. It's typically done as a course over a few months. For men noticing thinning or an unhealthy, flaky scalp, it's a gentle, non-invasive way to improve the environment your hair grows in. It won't reverse genetic hair loss on its own, but a healthy scalp is the foundation for everything else.
Aqualyx for stubborn fat
Aqualyx is an injectable fat-dissolving treatment based on deoxycholic acid, a substance that breaks down the membrane of fat cells so the body can clear them away naturally. It's used for small, stubborn pockets that don't shift with diet and exercise, the classic examples being a double chin or love handles. It's delivered as a course of sessions spaced a few weeks apart, and there's usually some swelling and tenderness for a few days afterwards. For a man who wants a sharper jaw and neck but doesn't have enough of a defined chin because of a small fat pad underneath, combining chin filler with Aqualyx can work particularly well.
Emsella for pelvic floor strength
Emsella is a very different sort of treatment, and one men rarely realise is available to them. It's a chair that uses high-intensity focused electromagnetic energy to trigger thousands of powerful pelvic floor contractions in a single session, far more than you could ever do with exercises alone. You sit on it fully clothed for around half an hour. It's used to strengthen the pelvic floor and to help with issues like bladder control, which can affect men too, particularly with age or after prostate surgery. There's no downtime at all. It's a discreet, comfortable option for something a lot of people never think to raise.
Common questions about aesthetic treatments for men
Will people be able to tell I've had something done?
That's the concern nearly every man raises, and the honest answer is that done properly, no. Our aim is for you to look refreshed and less tired, not different. We tend to treat conservatively, review, and add more only if needed, rather than doing too much in one go. Colleagues are far more likely to ask if you've been on holiday than to guess you've had treatment.
Does it hurt, and how much downtime is there?
Anti-wrinkle injections use a very fine needle and are over quickly, with little more than a brief pinch. For filler we use products containing local anaesthetic and can apply numbing cream first, so most people are comfortable. Bruising and a little swelling are the main things to expect, and men can bruise slightly more because of the richer blood supply in the skin. Most people go straight back to work. We usually suggest avoiding heavy exercise, alcohol and very hot environments for a day or two.
How long do the results last?
Anti-wrinkle injections typically last around three to four months to begin with, often longer with repeated treatment. Structural filler in areas like the chin and jaw tends to last a good while, often up to a year or more, because there's less movement in those areas than in, say, the lips. We'll talk through a maintenance rhythm that suits you rather than pushing you to come back sooner than you need.
Is filler safe, and what if I don't like it?
In experienced medical hands, and with the hyaluronic-acid fillers we use, it's a very safe treatment. One reassurance we give patients is that these fillers can be dissolved with an enzyme if you're ever unhappy or if there's a complication, so nothing is permanent. All of our injectors are doctors, which matters most in the rare event something needs managing.
How do I know which treatments I actually need?
You don't have to, and that's the point of the consultation. Most men arrive knowing they look tired without knowing why. Our job is to assess the whole face and profile, explain what's driving the effect, and suggest the smallest, most sensible plan to address it. Sometimes that's a single treatment. Sometimes it's a staged package like the one described here.
