Secret Pro RF Microneedling: How It Tightens Skin and Refines Texture

WF The Wellface Team Published 21 October 2023 · 9 min read

There is a moment, usually somewhere in your late thirties or forties, when the mirror starts telling a slightly different story. The skin along the jaw feels less defined. Pores that were once barely there look a little wider. The whole face seems softer, in the way that has nothing to do with a good night's sleep. It is skin laxity, and it happens because the collagen and elastin that once held everything taut begin to slow down and thin out.

Secret Pro is one of the treatments we reach for when a patient wants to address that change without surgery. It is a Cutera platform that combines two technologies in one device, and the part featured in this treatment is radiofrequency microneedling. In a recent reel, one of our patients came in for her second session, and the difference from the first was already clear: firmer skin, brighter tone, tighter pores and a more sculpted jawline. This is the kind of result the treatment is built for.

Below we explain how radiofrequency microneedling actually works, what a full session on the face and neck involves, why we sometimes go deeper under the chin, and what to expect afterwards. If you have been researching skin tightening and feel a little lost in the jargon, this should give you a clear picture.

What Secret Pro actually does

Secret Pro from Cutera is a dual-modality system. It houses a fractional CO2 laser for resurfacing the skin's surface and a radiofrequency (RF) microneedling handpiece for working deeper in the dermis. The treatment shown here uses the RF microneedling side, which is the tool of choice when the main goals are firmness, laxity and texture rather than resurfacing alone.

Here is the principle behind it. A cluster of very fine, insulated microneedles is passed into the skin at a precise, controlled depth. As they reach that depth, they release radiofrequency energy directly into the dermis, the deeper layer where collagen and elastin live. That energy gently heats the tissue. Controlled heat in the dermis triggers the body's natural repair response, and over the following weeks the skin lays down fresh collagen and elastin and remodels the existing framework.

The clever part is the combination. Microneedling on its own creates tiny channels that prompt healing near the surface. Radiofrequency on its own heats tissue but struggles to reach the depth where laxity really sits. Put them together and you deliver heat exactly where it needs to be, deep in the dermis, while sparing the surface from unnecessary trauma. Because the needles are insulated and the energy is released at the tip, the upper layers of skin stay relatively protected, which is part of why downtime is shorter than you might expect for something working this deep.

One practical advantage worth knowing: RF microneedling does not rely on light being absorbed by pigment the way some lasers do. That makes it a sensible option across a wide range of skin tones, including deeper skin types that are sometimes told to avoid more aggressive resurfacing.

The concerns it targets

The reel focused on two things, and they are the two Secret Pro handles particularly well.

The first is skin laxity and sagging. As collagen production falls, skin loses its spring and starts to descend, softening the jawline and the contour of the neck. By stimulating new collagen deep in the dermis, RF microneedling helps the skin contract and firm from within. Patients often describe the effect as their face looking more "lifted" or "snatched" along the jaw, even though nothing has been cut or pulled.

The second is skin texture and pores. Enlarged pores, rough patches and general dullness tend to travel together, and they respond well to the remodelling process. As the dermis rebuilds, the overlying skin looks smoother and more refined, pores appear tighter, and the whole complexion tends to look brighter and more even. The treatment is also used for acne scarring, stretch marks and fine lines, because the mechanism, prompting the skin to rebuild itself, is the same regardless of the specific concern.

In our patient's case, both were addressed in a single visit. We treated the full face and carried the treatment down onto the neck so the two areas would heal and improve in step, rather than leaving a firmer face sitting above an untreated neck.

What a full session involves

A session is methodical, and understanding the sequence takes a lot of the mystery out of it.

Preparing the skin

The skin is cleansed thoroughly, and a topical numbing cream is applied and left to work for a while before we begin. This makes the treatment very comfortable. Most patients tell us it is far more tolerable than they had braced themselves for.

Working across the face

Once the skin is numb, the handpiece is passed methodically across each area. In the treatment shown, we did at least three passes around the eyes, under the brows, across both cheeks, the forehead, the nose, the chin and the upper lip. Different regions of the face have different skin thickness and different needs, so the depth and energy are adjusted as we move around.

The under-eye area is a good example. The skin there is delicate but also prone to crepiness and hollowing, so we often go a little stronger and add a couple of extra passes to give it the attention it needs. Because this was our patient's second session, we were able to increase the intensity across the board. She already knew what the sensation felt like, and going a touch stronger the second time around helps build on the collagen response from the first.

The sensation

It is worth being honest about how it feels. It is not painful for most people, but certain spots can make you twitch a little, usually the areas over bone where the skin sits close to the surface, like the forehead or along the jaw. The settings can be dialled up or down in real time, so if a particular area feels sharp, we simply adjust. Nothing about the treatment is fixed; it is tailored as we go.

Going deeper under the chin

The jawline and the area beneath the chin got particular focus in this session. We increased the radiofrequency settings there to push hard on skin tightness, because this is where laxity shows most obviously. We also went a little deeper than usual under the chin, not only to firm the skin but to target a small amount of fat in the submental area. RF energy at the right depth can help reduce a modest fat pad while tightening the skin above it, which is why the jawline often looks more defined after treatment. We then carried the passes across the whole neck to lift the overall skin quality and let it match the face, so the finished result looks uniform rather than treated in patches.

Downtime, aftercare and results

Immediately afterwards, the skin looks flushed, rather like mild sunburn, and feels warm and a little tight. Some patients notice tiny pinpoint marks where the needles passed, and occasionally a little swelling, especially around the eyes. This is entirely normal and it is a sign the treatment is doing its job.

The redness usually settles over a few days, and most people find it has largely calmed within two to three days, sometimes sooner for a lighter treatment and a little longer after a stronger one like this. Many patients feel comfortable returning to work and normal life quite quickly, particularly if they are happy to skip make-up for a day.

Good aftercare protects the result. In the first days we recommend keeping the skin clean and well moisturised with something gentle, avoiding active ingredients like retinoids and acids until the skin has settled, staying out of direct sun and heavy heat such as saunas and hot workouts, and being diligent with a high-factor SPF. Freshly stimulated skin is more sensitive to UV, and protecting it helps the collagen do its work.

As for results, there is an immediate plumping and glow from the initial swelling and stimulation, but the real change is gradual. Collagen remodelling takes weeks, so the skin keeps improving over roughly the following one to three months. That is also why Secret Pro is usually recommended as a short course rather than a one-off. Our patient was on her second round precisely for this reason: each session builds on the last, and the firming and refining effect compounds. Once the initial course is complete, the results can last a good while, and many patients return for an occasional maintenance session to keep collagen ticking over.

Common questions about Secret Pro

Is Secret Pro the same as ordinary microneedling?

No. Traditional microneedling creates tiny channels in the skin to prompt healing near the surface. Secret Pro adds radiofrequency energy delivered through the needle tips into the deeper dermis. That heat is what drives the tightening and firming effect, which is why it reaches concerns like laxity that standard microneedling cannot address as effectively.

How many sessions will I need?

Most people benefit from a short course, often around three sessions spaced a few weeks apart, though the exact number depends on your skin and your goals. The patient in the reel was on her second round and had already seen firmer, brighter skin and a more defined jawline after the first. Your practitioner will recommend a plan at your consultation.

Does it hurt?

For the vast majority of patients it is comfortable rather than painful, thanks to the numbing cream applied beforehand. You may feel some areas more than others, particularly over bone, and the settings can be adjusted throughout the session if anything feels sharp.

Can it really help a double chin?

It can help with a modest amount of submental fat. By treating deeper under the chin and raising the radiofrequency settings, the energy can reduce a small fat pad while tightening the overlying skin. It is not a replacement for treatments designed purely for larger fat deposits, but for the right candidate it sharpens the jawline noticeably.

Is it safe for darker skin tones?

Radiofrequency microneedling does not depend on pigment to work, unlike some light-based resurfacing. That generally makes it a suitable option across a broad range of skin tones. As always, a proper assessment beforehand is the best way to confirm it is right for you.
